Periareolar Top Surgery
The donut method — scars hide at the areolar border
Also called the donut or circumareolar technique. Tissue is removed through concentric incisions that circle the areola, which is resized in the same step. The nipple stays attached and keeps its own blood supply.
Who it suits
Small to medium chests with good skin elasticity, where an intermediate amount of skin needs to come out.
Advantages
Minimal visible scarring — the marks sit at the border between areola and skin and are hard to spot once faded. Nipple sensation is far more likely to be preserved.
Trade-offs
Limited tissue removal compared with double incision, a risk of areolar widening or puckering over time, and a higher revision rate than the larger technique.
